Wisconsin Horizon Wellness is developing services for individuals who receive Wisconsin Medicaid and participate in long-term care programs such as IRIS, Family Care, and Family Care Partnership.
Our goal is to provide individualized behavioral health, therapeutic, and wellness services that complement each person's goals and person-centered plan.
Services
Depending on program eligibility, authorization, and provider requirements, services may include:
Counseling and therapeutic services
Behavioral health support
Emotional regulation and coping skills
Wellness support
Mental health support
Recovery support
Goal development
Skill building
Community-based support
Coordination with the individual's care team
IRIS
Include, Respect, I Self-Direct (IRIS) allows eligible individuals to have greater control over the services and supports they receive.
Wisconsin Horizon Wellness intends to work with eligible participants and their care teams to provide appropriate services based on the individual's approved plan.
Family Care & Family Care Partnership
Wisconsin Horizon Wellness also plans to serve eligible individuals participating in Wisconsin's managed long-term care programs.
Services will be individualized around the person's needs, goals, strengths, and level of support.
